with respect to the advancement of rank, if a captain has 5 persons under his or her command, could they all become commander level? If not perhaps the language could be modified to indicate why a person might be 'stuck' in the rank of ensign is not because they aren't working hard, but merely that there are no open positions above them within their squadron. If a division becomes full, perhaps a new squadron could be formed...and assignments of this new squadron will be made by the relivant admirals (at which time a VERY well-achieving ensign who hasn't been allowed to advance merely because of filled ranks might be granted a promotion to lieutenant commander for instance?)
These are just suggestions and thoughts. I submit them to you Fleet Admiral for your consideration.

As for your question, no they can't all be commanders, when someone is more hard working he moves up the ladder, if there is no more room he has to change squads. If a squad is full we make another. That was implied when i discussed commodore rank that it is possible.
The most active and hard working friendly members will get the highest ranks, i applaud effort but i give rank based on what results you actually bring in. the higher the rank the higher your responsibility. Nobody forces you to be a commander or an admiral. I give it and i take it away when needed to assure the optimal functioning of the fleet.
This must all seem like i am asking a lot of work but it isn't. Talking to fleet members if there is a problem, you do that anyway! Recruitment for 10 minutes a day is not that high a sacrifice either.
